Scott McTominay will step away from Napoli duty in the coming days to undergo heart surgery after the club confirmed the discovery of a “mild benign arrhythmia”.
The 29-year-old midfielder, a cornerstone of Napoli’s recent resurgence since his £25 million move from Manchester United in 2024, is set for a corrective cardiac procedure in Italy but has been assured his career is not under threat.
Napoli move quickly to calm fears
Napoli released a detailed medical update on Tuesday, outlining both the diagnosis and the plan.
“Following the onset of a mild benign arrhythmia, Scott McTominay will undergo corrective surgery using a cardiac arrhythmia ablation (PFA) in the coming days. He will be available again after the international break,” the statement read.
Club doctors have stressed the condition is relatively common among elite athletes and have downplayed any long-term risk, a crucial message for a fanbase that has watched McTominay become one of the side’s leaders on and off the pitch.
His last appearance came in a shock home defeat to Como at the weekend, a jarring result that only added to the sense of unease around Napoli’s start to the season.
Allegri’s early problems deepen
Even before the heart issue was made public, Massimiliano Allegri had hinted that his midfield general was not fully fit.
“McTominay had some problems with blisters, so he’s been training on a limited basis. Hopefully, we can get him into the best possible condition as soon as possible,” the Napoli manager said after the loss to Como.
The news that the Scot will now miss a crucial stretch of games lands at the worst possible time for Allegri, who has walked into a storm since taking over from Antonio Conte after last season’s failed title defence. Napoli finished runners-up in Serie A, then tore up the script in the summer.
Romelu Lukaku, Giovanni Simeone and Miguel Gutierrez all departed. In response, the club doubled down on rebuilding, sealing the permanent signing of Rasmus Hojlund from Manchester United and bringing in Chelsea defender Benoit Badiashile on loan.
The overhaul has yet to click. Two league games, one win, one defeat, and a place in mid-table: Napoli sit tenth, still searching for rhythm and identity. Losing their midfield anchor now only complicates that search.

McTominay’s impact in Italy has been emphatic. Since arriving in 2024, he has driven Napoli to a Serie A title and become a mainstay in Allegri’s engine room.
The numbers back it up. He has already made 82 appearances and scored 27 goals for Napoli, a remarkable return for a player nominally deployed in midfield. For Scotland, he stands on 73 caps and 15 goals, underlining his status as a central figure for club and country.
That influence is exactly what Napoli will be without as they enter a brutal run of fixtures.
Brutal schedule without their anchor
The timing could hardly be harsher. Between now and the international break, Napoli face four matches that could shape the mood of their season.
First comes a domestic heavyweight clash with reigning Serie A champions Inter on Saturday, a fixture that already feels like an early measuring stick for Allegri’s new project. Days later, the stage shifts to Europe and a high-pressure Champions League meeting with Arsenal.
Then come league tests against Bologna and Fiorentina, games that may not carry the glamour of Inter or Arsenal but will say plenty about whether Napoli can claw their way back towards the top of the table.
McTominay will miss the lot.
Napoli insist the procedure is straightforward and the outlook positive. If his recovery runs to schedule, Allegri could have his midfield leader back in time for the league clash with Frosinone on October 10.
By then, the question will be simple: will Napoli still be within striking distance of the summit when their enforcer returns, or will this early-season turbulence have already altered the course of their campaign?
